PRIVACY NOTICE

Medialpha Laboratories Inc. is fully committed to protect our patients’ and collaborators’ privacy. This privacy notice describes the details of our commitment.

Definitions

Personal information: Personal information is referred to information that identifies you or could reasonably be used to identify you and which is submitted to and/or collected by Medialpha’s websites, softwares, or other media in the course of our commercial activities. Examples of personal information include your name, your postal address, email and your telephone number.

Personal health information: Personal health information is referred to information that provides us information about your health status or that could reasonably be used to identify your health status and which is submitted to and/or collected by Medialpha’s websites, softwares, or other media in the course of our commercial activities. Examples of personal information include your RAMQ number, medical requisition and personal health related comments.

Can my personal information be used for commercial purposes such as Marketing?

• Yes, only your personal information could be used for marketing purposes.
• The marketing may be performed by Medialpha directly and/or a third party company to whom Medialpha enter in a contractual agreement to conduct such work on their behalf. Any third party company contracted by Medialpha is subject to the same rules Medialpha when it comes to the privacy of your information.
• Marketing techniques that could be used include the use of cookies, email marketing and re-targeting.

What personal health information is collected by Medialpha and for what purposes?

Medialpha collects the following personal health information via its platform QuickLab:
• Name of the patient
• Date of birth
• Legal ID number, such as RAMQ number or driver’s licence
• Address of the appointment (personal or work)
• Comments that patients willingly provides, such as small veins or fear of needles
• Requisition by the prescriber
• Next appointment with the physician, if patient willingly provides this information

All the information above is used to generate the following documents:
• Invoice for the patient
• Documentation for the laboratory about the analysis
• Documentation for the mobile professional about the appointment and instructions of how to handle samples collected during the appointment.

How does Medialpha use my personal health information?

Aside from the usage specified in the above statement, Medialpha does not share, sell or make available in any way patients’ personal health information to third parties.
Medialpha uses some personal information (not health related information), such as emails for example, for marketing purposes.

Who else has access to my personal health information?

Only the user that took patient’s appointment and the mobile professional that goes to the patient’s appointments can view view personal health information collected in QuickLab.

Why does Medialpha collect my credit card information?

Medialpha collects patient’s credit card information to pay for the appointment. The payment is scheduled 1 day after the appointment.
Medialpha does not store credit card information in the QuickLab system. Once it is processed to the payment platform, the credit card information disappears from the screen and can no longer be accessed by the users of QuickLab.

Is my personal health information disclosed to anyone outside of my circle of care?

No.

How is my personal health information protected?

Medialpha applies best practices for physical, administrative and technical security. This includes, but not limited to:
• Secure HIPAA compliant datacenters
• Encryption of personal health information in the database and in transit
• Access logging to the system
• Role based access control
• All users are trained and commit to comply with privacy and security policies
• Anonymous data extractions

How do I access and correct my personal health information?

Patients can see their personal information in the appointment confirmation email, and partially in the invoice. The patient has the option to opt out of Medialpha’s system by clicking a link in the received emails. To correct any personal information, patients can contact our Chief Compliance Officer at privacy@medialpha.ca.

Do you need my consent for the collection, use or disclosure my personal information?

When you use our service, we rely on your implied consent to collect, use and disclose the information for the purposes described above. We will not collect, use or disclose your information for any other purpose without your express consent. If you have any questions or concerns about our use of implied consent, please contact our Chief Compliance Officer at privacy@medialpha.ca.
